Manager Terry Francona said Ellsbury could return for Monday's game at Texas but indicated the Red Sox would treat his bruised back with caution.
"He's pretty sore," Francona said before Sunday's game against Kansas City. "When he's able to get in there and be comfortable to play, we'll get him back in there. Getting him back in there too early doesn't do anybody any good."
Royals reliever Everett Teaford drilled Ellsbury in the back during the eighth inning of Boston's 7-1 win Friday night. Ellsbury had appeared in 123 of Boston's 125 games before Sunday, posting a .313 batting average with 22 home runs.
"When he's ready, he'll get in there," Francona said. "Believe me, he wants to play. He's been playing every day."
The Red Sox's won Sunday's game, 6-1.
